چکیده

ارتقاء و به روز رسانی دانش پزشکی یکی از اصول حرفه ای در علوم پزشکی می باشد. مطالعات نشان می دهد که اجرای روش های سنتی و سخنرانی، فاقد اثربخشی و موجب دل زدگی، خستگی و نارضایتی شرکت کنندگان می شود. لذا با لحاظ نمودن اصول آموزش بالغین و با هدف ارتقای کیفیت خدمات بالینی، افزایش ایمنی بیمار و یادگیری از خطاها، کنفرانس های موربیدیتی و مورتالیتی در قالب دوره های بازآموزی ماهانه و با رویکرد آموزش بین حرفه ای برنامه ریزی و اجرا شد. در این مطالعه ی شبه تجربی طرح موارد با نظر کمیته ی مورتالیتی و بر اساس فرمت SBAR انجام شد. ابزار پ‍ژوهش پرسش نامه ی محقق ساخته، پیش آزمون، پس آزمون و مصاحبه ی نیمه ساختارمند بود. 96% از 202 نفر شرکت کننده، جذابیت کنفرانس های مورتالیتی و موربیدیتی را بسیار خوب و خوب ارزیابی نمودند. اختلاف میانگین نمرات پیش آزمون و پس آزمون معنادار (0001/0P=) و مبین افزایش دانش فراگیران است. در تحلیل محتوای داده های کیفی ارتقاء در سه طبقه ی مهارت های بین حرفه ای، عملکرد مبتنی بر سیستم و آموزش بین حرفه ای در مصاحبه ها تشخیص داده شد. با توجه به نتایج مثبت این مطالعه و رضایت مندی 95% از شرکت کنندگان از نظر جذابیت برنامه و عدم اختلاف در رضایت مندی بین گروه های مختلف حرفه ای و رده های سنی و نیز ارایه ی خدمات مراقبتی_درمانی در قالب تیم های حرفه ای و با توجه به استقرار نظام حاکمیت بالینی و بهبود کیفیت در مراکز درمانی، اجرای کنفرانس‌های موربیدیتی و مورتالیتی در قالب برنامه های بازآموزی حرف پزشکی توصیه می-شود.

چکیده [English]

Designing based on principles of adult learning is an essential part of medical education. Accordingly, mortality and morbidity conference designed with inter professional education approach and CME score. After implementation, its satisfaction and acceptability and educational impact was assessed. This is a triangulation study. Monthly morbidity and mortality conferences with CME score was held during one year in 22ndBahman teaching hospital. The tools that be used were pre and post-tests and a satisfaction questionnaire developed by the researcher. Also a semi structured interview was done with 10 people who were participated in 5 or more conferences. Data was analyzed by SPSS software. Content analysis was done about interview transcripts. 202 medical professional people with average 37years old include 71%female and 29% male. 96% of participants have assessed attractiveness of these programs as very good. Pre and post-tests scores showed significant differences (p-value≤0.0001) and indicate increased of the learners' knowledge. In qualitative data analysis,95% of participants stated that will change their behavior, clinical reasoning and decision making. Also they found improvement in professional interpersonal communication, awareness of system based practice, effectiveness of inter professional education and learning from real errors, patient safety and quality of care as the positive outcome of it. High satisfaction that expressed by these medical professionals confirmed the effectiveness and fairness of the conferences.
